comments and requests. To help us continue to achieve this policy
objective, we ask you to rate and comment on your INSTRUCTOR,
and on the COURSE itself. The information that you provide will
assist us in making any necessary modifications.
The rating system for each of your answers is on a 5 - 1 basis, with:

  5--->Excellent    4--->Very good    3--->Acceptable

  2--->Not Acceptable  1--->Poor
